Hi guys,
Im on a short stay ward for my Cardio placement. I couldn't have asked for a better place to be. Its really giving me insight about the programs that are available when a patient leaves hospital. It is also giving me insight about the importance of preventing readmission. These programs are not only important for a patient physically but they are also important to a patient mentally and socially. I challenge you all; the next patient you see wheather it is inpatient neuro or out patient musculo, look up what other programs they can be involved in to help with their condition either physically or mentally. I bet you will be surprised at how much stuff is out there waiting to be utilized.

Great point big man. i'm on cardio as well and it's really interesting to see what is out there for patient's following discharge. On my respiratory medical ward, patients can either discharged to an 8-week pulmonary rehab program, to physio in the home (home link), to outpatients, to community physiotherapy classes, or to self management of chronic disease programs. I'm sure there are even more programs that i have still yet to discover. if we get this part of patient management right, hopefully there won't be those re-admissions that we commonly see
Hi Shadi. I am on cardiopulmonary patient as well and I totally agree in what you are saying. We definitely need to be aware of the other programs available for the better treatment/management of our patients. I think it is really important to give adequate support to our patients before discharge, referring them to the programs in community that best suit them and their condition. I’m also noticing how important is to liaise with other professionals, such as social worker and OT prior discharge. It makes a lot of difference in the patient’s well being and safety. Sometimes they are extremely anxious with their home condition that can affect on their symptoms. I am finding very interesting to be able to participate on those meetings with all professionals involved on the patient’s care and act actively on the discharge planning.
